The Case Manager (female only) will serve as a liaison between the female house members and external resources, helping them navigate their journey toward sobriety and independence.
Average 10 – 25 hours per week, per diem
The duties of a Case Manager, including advocacy, education, and service are as follows:
● House Member Meetings Facilitation
Meet with each house member on a weekly basis to evaluate their progress, identify immediate and long-term needs, and adjust action plans as necessary.
● Case Management
Assist house members in creating and following individualized recovery plans, which include setting achievable goals related to health, employment, education, and life skills.
● Resource Coordination
Connect house members with community resources, such as healthcare services, job placement programs, mental health support, and educational opportunities.
● Documentation & Reporting
Maintain detailed records of house members\’ progress, including meeting notes, goals, and resource utilization. Ensure all documentation is compliant with organizational
policies.
● Crisis Intervention Support:
Provide immediate support to house members in crisis situations, ensuring they have access to appropriate care and assistance.
